**LOCATION**: Stockport

**SALARY **:Up to £23,000 depending on experience

The company is searching for a driven, organised Care Co-ordinator to liaise with clients and healthcare professionals to ensure the organised regulation of the premium quality care company.

Our client will invest in your training and provide ongoing career development for the successful ambitious, caring and organised Care Co-ordinator. You will also be offered mentorship from another member of the team already in the role.

**ABOUT THE ROLE**:

- 37.5 hours per week
- Liaise with the team, health care professionals and clients
- Manage daily communications between your CareGivers
- Build trusted relationships with your CareGivers / Health Professionals
- Manage rota’s, observations and shadowing
- Complete care plans
- Service reviews
- Solving problems
- Sharing company goals and embedding a forward thinking caring culture
- Occasional Care calls if your CareGivers are unavailable

**WHAT WE EXPECT/NEED FROM YOU**:

- Previous Health or social care experience would be beneficial although not essential
- Extremely organised and can manage your time effectively
- Excellent communication skills
- A resilient and empathetic nature
- Supportive, in order to build relationships with both your staff and clients
