Care at Home Group is growing by the day, we have recently introduced our very own Rapid Response team alongside an oncall system. We are excited to recruit for a Care Coordinator that would primarily schedule the Rapid response and oncall team. The purpose of the Rapid Response team is to facilitate and deliver intermediate domicilairy care for clients being discharged from hospital and other intermediate care settings into their home. Care at Home Group have introduced the Rapid Response team to provide emergency cover to both CAHG branches and other home care providers accross the North West. The Rapid Response team will be required to cover a range of optional shifts to ensure the support is provided until either CAHG or other agencies are ready to take over the packages. As the Care Coordinator you will support to manage the Rapid Response team, ensuring all relative compliance is completed correclty; including: supervisions, team meetings and appraisals. This role is extremely fast-paced and requires a calm and positive individual to complete this role successfully. - **Duties and responsibilities**:_ - Allocating the intitial one to two weeks of rotas until CAHG branches or other agencies are ready to take over the package of care. - Providing feedback to CAHG branched or other agencies on the package of care and the service users needs to support the agencies to complete their care plans. - Keeping up to date with support workers compliance including, spot checks, medication competencies, supervisions and appraisals. - **Essential Requirements**:_ - Previous experience within agency support, rapid response work, care coordinator is essential. - Excellent Communication and organisational skills. - Ability to organise and prioritise workload within a fast-paced environment. - Willing to take part in care calls when needed - a requirement to support with weekend cover when needed. - Full UK driving license. - Willing to complete the Rapid Response On call system. **Benefits and Incentives** - Referral Scheme - CAHG Business Induction Programme - Dress down Fridays - "Grow your Own Footsteps"- Progression pathways - Mental Health Support & Advice **Job Types**: Full-time, Permanent **Salary**: £22,000.00-£25,000.00 per year **Benefits**: - Company pension - On-site parking - Referral programme - Wellness programme Schedule: - 8 hour shift - Monday to Friday - Weekend availability Supplemental pay types: - Bonus scheme - Loyalty bonus **Experience**: - Care Coordinator: 1 year (required) - Rapid Response: 1 year (required) Work Location: One location
