**The role**:
An exciting opportunity has arisen for experienced social workers, based within the award-winning Integrated Hospital Discharge Team (Transfer of Care Hub) at Warrington and Halton Hospitals.
The Integrated Hospital Discharge Team consists of qualified social workers, community care workers, qualified nurses, occupational therapists, discharge case managers, discharge facilitators and administrative staff who all work closely with the multi-disciplinary teams based on the individual wards within a busy hospital setting.
The team supports the wards to facilitate safe and timely discharges of patients from the hospital, working within the Discharge to Assess model. To achieve this they use a wide variety of personalised support, assist people to promote their independence, support people with complex needs and often changed situations to ensure they have the maximum level of choice and control available to them, promoting the principles of The Care Act and Mental Capacity Act.
You will be supported throughout your time with us by dedicated teams from Practice Development, Leadership and Learning Development. This support includes access to specific programmes, e-learning and bespoke CPD funding to help you access university modules linked to your goals and objectives to take your career in the right direction.
DBS clearance is required.
The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
