**The role**:
We provide urgent **(in two hours)** assessment and support **(in two days) **for a short time, to help people recover and retain independence.
We ensure people are supported to:
- remain at home when things start to get challenging or difficult
- recover after a fall, accident, acute illness or operation that would otherwise put them at risk of going into hospital
- reduce the risk of being admitted to hospital
- return home more quickly after a hospital stay
What we do
Our team is made up of nurses, therapists and social care workers, who are part of a wider integrated intermediate care model who can offer on-going support as required.
We triage, assess and support people during a crisis. People eligible for the service are:
- adults aged 18 and over who live in or registered with a GP in Warrington
- those at risk of hospital attendance/admission
- people who are medically safe to be treated/cared for in a community setting
- those in need of assessment/intervention within two hours (and are safe to wait for up to two hours)
As a Community Care Worker (Social Care Assessor), you will be a valued member of the Urgent Community Response team working with health and social care colleagues, carers and families and other statutory and voluntary agencies to identify and plan the support people require in crisis situations with the aim to support individuals to remain safe at home and prevent hospital admissions or the need for emergency services being called. Our team is solution focused placing the needs of the person at the heart of our decisions, ensuring they have the support they require to remain safe and as independent as possible in their own home.
This role involves assessing patients for social care needs, ongoing care/support and signposting and making ongoing referrals where needed. Administrative work in triaging and screening new referrals for people, whilst working together with the multi-disciplinary team to identify the best pathway for people in crisis.
Collaborative working and the ability to create and maintain effective working relationships with a range of health and social care colleagues is an essential element of the role.
Experience of working with older people and/or adults with a disability will be an advantage in this role. The successful applicant will be able to demonstrate excellent communication skills and must be flexible, reliable and committed to personal and service development.
DBS clearance is required.
The ability to converse at ease with customers and provide advice in accurate spoken English is essential for the post.
