Continuity of Care Services are looking for a Care Coordinator to join our Domiciliary Care Company in Maidstone.

Continuity of Care Services has been incorporated since 2016 looking after children and adults in their own homes throughout Kent.

The role of a Care Coordinator has arisen due to the company growth and for the on-going support for the management team.

The role is fundamental for ensuring all our Care Assistants are fully supported and families are receiving the best possible care, in line with our mission statement above.

The Care Coordinator is line manager to our Field Care Supervisor, Head Care Assistants and Care Assistants.

**Key aspects of the Care Coordinator role**:

- Scheduling staff rotas' of care for our families
- Supporting the Field Care Supervisor and Head Care Assistants in Complex rotas'
- Being a Line Manager to the Field Care Supervisor and Head Care Assistants.
- Approving Holiday
- Managing sickness and spotting trends.
- Delivering back to work interviews
- Meeting all families and delivering care as and when required
- Carrying out Risk Assessments
- Taking part in On call rota

**Experience/ Qualifications**
- Have completed Level 2 in Health and Social Care
- Have completed or working towards Level 3 in Health and Social Care
- Good Time Management
- Effective Communication for staff and our families
- Has a driving licence and access to a car
- Must be able to drive to the organisations operational areas.
