Job Overview
We are seeking a dedicated and compassionate Care Coordinator to join our team. To provide leadership to the care staff within the service. To provide care in accordance with current best practices. To supervise the care services in accordance with agreed standards.
Responsibilities
- Ensure service users are at the heart of the care delivery and their wishes and preferences enhance their wellebing
- Supervise staff to esnure efficient running of the service
- Promote and protect the welfare of the service users
- Develop effective working relationships
- Work with multidisciplinary teams
- Ensure all visits are allocated in a timely manner and coordinate new referalls
- Carry out assessments and reviews for exisitng and new service users
- Carry out employee supervisions
- Ensure governance and compliance standards are maintained
- Carry out reviews of service users and update documents
- Work to implement new business streams into the business
Skills
- Proven office experience with strong organisational skills
- Excellent verbal and written communication abilities, with a focus on empathy and active listening
- Strong problem-solving skills, with the capacity to handle challenging situations effectively.
- Proficiency in using Nourish computer software for documentation and communication purposes
- A genuine passion for providing high-quality care and support to individuals in need.
Job Type: Full-time
Pay: £26,499.72 per year
