Care Coordinator

**Job Overview**
**Responsibilities**
- Coordinate care plans for clients, ensuring all necessary services are provided efficiently.
- Act as the primary point of contact for clients and their families, addressing inquiries and concerns promptly.
- Maintain accurate records of client interactions and care plans within the office system.
- Collaborate with healthcare providers to ensure comprehensive care delivery.
- Schedule appointments and follow-up visits for clients as needed.
- Monitor client progress and adjust care plans in consultation with relevant stakeholders.
- Provide administrative support within the office, including filing, data entry, and maintaining documentation.
- To provide leadership to care staff within the service
- To provide care in accordance with current best practices, according to policy and procedures, agreed standards, legislative requirements and relevant regulations under the direction of the Manager
- To supervise the care delivery within the service
- To maintain skills at a current level, and undertake such training and development as may, from time-to-time, be required to maintain practices as up to date
**Experience**
- Previous experience in a healthcare or social services setting is advantageous.
- Strong organisational skills with a keen attention to detail.
- Excellent communication skills, both verbal and written, to engage effectively with clients and professionals alike.
- Proficiency in using office software and systems for record keeping and scheduling.
- A compassionate approach to client care, demonstrating empathy and understanding in all interactions.
Car is needed for this role.
**Job Types**: Part-time, Permanent
Pay: £13.00-£15.50 per hour
Expected hours: 15 - 25 per week
