PURPOSE OF THE JOB:
Responsible to a Senior Practitioner within the locality neighbourhoods team, the post holder will work with Social Workers and Occupational Therapists to provide effective, customer focused, and fully accessible information and initial assessment for Hackney residents and their carers.

The post holder will provide a high standard of customer care through the provision of information, advice and assistance concerning council services and other services provided by statutory, independent and voluntary agencies to all callers. In doing so they will be responsible for recording customer contacts and carrying out initial conversations and in supporting residents to make connections with services.

A significant responsibility of the role will be carrying out non-complex social care assessments,using a strength based approach.

The post holders will act as Trusted Assessors providing simple equipment for older and disabled people to help them to maintain their independence. They will also be responsible for producing individually tailored plans for simple equipment, recommending and requisitioning Telecare and setting up social care support.

**Delivery**

1. Experience providing a customer service direct to the public, working to understand their needs sensitively and effectively, and providing information, resolving or re-directing queries on to other services as appropriate.

2. Carrying out strength based assessments for non-complex cases, identifying the outcomes that a person wishes to achieve and linking residents with support including community and voluntary sector resources.

3. Producing outcome focused care plans in partnership with service users and their circle of support.

4. Actively supporting and promoting Equal Opportunities and fair access to services.

**Knowledge of Adult Social Care**

1. Working knowledge of the Care Act and Mental Capacity Act and related legislation,

2. An understanding of Telecare and assistive technology and its impact on maximising independence.

3. Good understanding of the London multi-agency adult safeguarding policy.

4. Good understanding of the role of a Trusted Assessor.

5. Understanding of self assessment in adult social care.

6. Understanding of the needs and issues affecting people living in a diverse, inner-city community, and the impact this may have on communication, and the range of services available to support wellbeing and facilitate independence.

7. Good understanding of the legislation and Council policies covering data protection, information sharing and confidentiality.

**Technical skills**

1. Experience storing, retrieving and sharing confidential and private customer contact information in line with legislation and organisational policy and practice.

**Salary**: £18.00-£19.00 per hour
