**Essentials**:
- Previous experience conducting assessments with Adult Social Care
**Job Role**:
- Post for access and duty
- Acting as the first point of contact and reception service for any customers to Hackney Council’s Adult Social Care services
- To carry out an initial contact assessment to identify the needs of the service user
- To provide information, advice or signpost service users to a range of universal services and recommend and set up any telecare support as required based on the outcome of these assessments
