This is an exciting opportunity to work as part of our dedicated team working in partnership the Hywel Dda Community Children Nursing (CCN’s), in the provision of valuable support for vulnerable children with complex clinical, medical & physical needs enabling them to remain living at home, while providing essential respite for their parents / carers to support continue their caring roles.

**About this job**:
As a Children’s Clinical Support Worker, you will work closely with the team to provide 1:1 (occasionally 2:1) predominately overnight support in the home, and on occasion in residential respite settings. Support provided includes support with personal care, nutrition and fluids, manual handling, as well as undertaking specialised tasks as agreed and after appropriate training, for example, administration of medication by specialist techniques and assistance with oxygen therapy. The service you provide focuses on promoting the wellbeing, rights, and independence of the individual.

**Responsibilities**:

- All Adferiad Support Workers are given the qualifications, experience, and competencies commensurate with the levels of care they are expected to provide and in accordance with the Social Services Wellbeing Act. This includes supporting individuals if required with:

- All aspects of personal care including tracheostomy, ventilation, PEG/NG feeding, and any other child specific clinical need.
- Mobility and transfers - all staff are trained in accordance with the All-Wales Manual Handling Passport.
- Administration of prescribed oral, topical, or parenteral medication, including where applicable epilepsy rescue medication administration
- Basic physiotherapy/occupational therapy as directed.
- Escort /Accompany individual to school.
- Support with engagement with social activities including holidays away from the family home.
- Encourage play activities.
- Completion of detailed observations and reports in relation to individuals which can be shared with health professionals to ensure adequate care arrangements are in place from statutory/voluntary services to meet the individuals needs if continued care and ongoing support is required.
- Working to a rolling shift pattern over waking nights - rosters are created and managed to ensure continuity of support staff for individuals while minimising travel and promote good work/life balance for support staff.

**Person Specification**:

- All staff would be required to hold or complete an NVQ Level 2 (or equivalent) in Health and Social Care as part of their employment, in order to register with Social Care Wales.
- It is desirable though not essential that applicants have one or more of the following:

- Experience of providing care and support services within individuals own homes
- Experience of providing care and support services within a hospital or residential setting
- Experience of working with vulnerable children and / or adults.
