**Salary**: £21,507.25 pro rata per annum (actual salary: £17,206.20)

Adferiad Recovery operates domiciliary support services including domiciliary home care, children’s services, palliative care, and respite provision for Carers. Our services are delivered in line with our commitment to principles of empowerment, self-management, wellbeing, and recovery, whilst ensuring compliance with internal and external quality standards inspection and regulation (including CIW).

As a Care Planner you will be a key member of the team delivering Adferiad Recovery’s Pembrokeshire Domiciliary Home Care Service. This service is delivered in people’s own homes 1:1 (occasionally 2:1) though a dedicated team of workers supporting individuals with varying ages and support needs - including physical disability, mental health, learning disability, and frailty.

Support provided includes support with personal care, medication, manual handling, and social inclusion, whilst focusing on promoting the wellbeing, rights, and independence of the individual.

The post holder will work closely alongside our experienced Service Manager and Registered Manager in the delivery of Domiciliary Home Care in Pembrokeshire. As the Care Planner you will be responsible for promoting the wellbeing, rights, and independence of the individuals we support through the completion of detailed assessments, person centred Care & Support Plans, and Risk Assessments.

The Care Planner will also, under the direction of the Service Manager, be responsible for the day-to-day support, monitoring and evaluating of staff performance though the completion of supervisions and observations, while working to grow and develop this existing service and staff team.

**Main Duties and Responsibilities**:

- Processing of approved new referrals - following procedures for completing new assessments, risk assessments, and creating individual person-centred service delivery plans.
- Follow procedures for conducting 3 monthly reviews (or when required due to changes in individuals’ needs or circumstances), ensuring risk assessments and support delivery plans are kept up to date and are relevant - whilst promoting self-care and positive risk taking.
- Administration and maintenance of web-based care management system to record relevant staff and service user information (training provided).
- Providing support to the Service Manager & Registered Manager, in monitoring and evaluating staff performance though the completion of supervisions and observations and the provision of ongoing support to staff. There may be a requirement to work unsocial hours including evenings and weekends to support staff training and to carry out observations due to the working patterns of some staff.
- Participation in the out of hours “On Call” rota, providing a point of contact for staff and individuals receiving support outside of office operating hours.
- There will be occasions where you may be required to cover unplanned absence of support staff to ensure continuity of service for the individuals we support. Relevant training (including registering with SCW) will be provided and must be undertaken.

**Person Criteria**:

- There are no specific qualifications but there must be evidence of commitment to personal and career development relevant to the post.
- Experience of providing support to individuals with health/social care needs in an individuals’ own homes.
- Experience of care management processes such as assessing individual needs and completing detailed risk assessments.
- Full driving licence and full access to a vehicle for work purposes.
