Urgent Response Service [URS] is an integrated Health and Social care team consisting of SCC and Solent NHS staff. We provide a joined up working approach to maintain people’s independence, prevent admission and provide timely discharge from hospital. We operate 365 days a year 7 days a week and have an exciting opportunity available

**Urgent Response Service Coordinator - non-supervisory**

Starting Salary £27,852 (FTE) + 15% shift allowance + contractual car user payment

Completing visits to service user’s homes, which involves risk assessments, medication records and goal setting.

Be competent in recording information onto IT systems.

Accurate recording and appropriate communication skills required.

Case hold service users assessing and identifying ongoing care needs.

Knowledge of CQC regulations

Must be able problem solve and be a good team player

Commitment and reliability are important criteria for this role.

Applicants will be expected to visit covid-19 positive service users as and when required while following government guidelines and with correct PPE.

3 week rolling rota between 06.30 am and 22.00

All applicants must hold a clean UK driving licence and have access to a car (mileage and car allowance paid]
