**Swansea Bay Home Care Services is an established Domiciliary Care Provider, offering quality services to clients in their own homes in the Swansea area.**

We are now looking to employ an additional Co-ordinator to join our Team.

To provide supervisory support to a team of Seniors and Community Care Workers, offering guidance, support, and motivation. Enabling them to work within a service that supports the continuing assessment of individuals within their own home, which promotes and facilitates independence and assists with the identification of any long-term care requirements as appropriate.

To support the Registered Manager, maintain the CIW National Minimum Standards of Social Care Practice.

Must have supervisory experience within a Social Care Setting and be competent to undertake the day-to-day operations of the service in the absence of the Registered Manager. Have the ability to work as an individual and part of a team.

To undertake a combination of both supervisory and practical duties. To carry out quality monitoring visits to ensure that high standards of care are met. To carry out reviews for service users to ensure Personal Support Plans are kept up to date. To ensure all essential service user cover is maintained during times of sickness and annual leave. To take responsibility for Health and Safety aspects of the caring environment, work flexibly as part of a rota to ensure continuity of the service, will be required to work some weekends and evenings.

**Salary**: £21,840.00 per year
