A position of Care Coordinator became vacant resulting from the promotion of the current post holder to a manager.

**What does this Care Coordinator job involve?**:

- To visit individuals and to make reviews of the written care plans, examine the written records of medication administration, finance and daily log notes, kept by staff as required by the manager
- To carry out initial assessments?and meetings with individuals and Care Assistants
- To be on call duties, covering shifts if staff are unable
- To ensure files are up to date, for example, Care plans, risk assessments etc
- Responsible for resourcing new care packages and building on existing packages
- To be aware of the quality assurance policy of the company, in the provision of quality service to individuals
- Improving and maintaining the continuity of care for new and existing care packages
- Managing & Monitoring Care Planner to ensure Care workers are productive and use the system effectively
- Ensure that Care Assistants rotas are organised, correctly matched to people's needs and well-planned, taking into account travelling time, holiday cover and last-minute cancellations
- Ensure staff are supplied with PPE
- Responsible for cordially resolving customer queries in a timely manner within your area to maintain a positive Company reputation
- Contribute to the growth and service expansion targets set by the business
- Work well as part of the Choice Global team
- Follow Choice Global policies, procedures, and guidance at all times
- Take part in staff and customer meetings
- Attend training activities and appraisal and development meetings

- Good organisational and supervisory skills, so staff receive necessary support and individuals receive the services they expect
- Ability to initiate and support staff and to use own initiative and work alone or as part of a team especially in an emergency
- Numerical skills to support individuals in managing their money and buying shopping or paying bills when requested to do so
- Ability to keep written records in clear English about the care and support given to each individual, including help with medicines
- Commitment to respecting individuals' rights at all times including their rights to privacy, dignity and independence aswell as being non-judgemental
