**1. Organisational Context**

Dagenham, Dartford, Greenwich, Ashford and Kent. We work from the following locations; **Romford, Dartford, Tunbridge Wells, Greenwich and Ashford.**
Every night we provide supported **accommodation** for homeless people locally, providing young people with opportunities to develop the necessary **life skills** to find work and live independently. We also provide a wide range of services in the local community; these include children’s, youth and family work and health & wellbeing facilities.

**2. Role Summary**
This job description is generic and applies to all housing & sustainment coordinators. The duties will be determined by the individual needs of the users being supported. The context in which support is provided may vary according to the users needs. This may include site-based support, housing management and floating support.
To provide housing related support and coordination to service users and customers to enable them to maintain their tenancies and to achieve their aspirations for independent living. The service operates on a 24/7 model so flexibility for occasional over-night based work may be required.

- their say in the things we do. _Deliver individual and group activities that provide opportunities and experiences where residents may learn and develop life skills in a safe, accepting environment. Assist service users to engage in education, employment and training (EET) and to maintain their own tenancy.
**3. Main Duties & Responsibilities**
- Effectively manage a caseload that provides constructive guidance to clients through dynamic support planning, proactive in preventing rent arrears building-up and to ensure that resident have the opportunity move-on in a positive manner.
- Ensure that the highest standards of delivery are obtained, towards our mission, of the positive transformation in the lives of young people.
- To provide housing related support to service users and customers to enable them to maintain their tenancies and to achieve their aspirations for independent living.
- Carry out and tailor one to one and group based interventions to service user’s needs.
- Identify users’ individual support needs and, through consultation and agreement with the user (and, where appropriate, their Care Manager), devise and implement suitable support plans to ensure that identified support needs are met.
- Encourage and support tenants to live as fully and independently as possible within the local community, providing information, emotional, organisational and practical support and training as appropriate.
- Carry out risk assessments as and when necessary due to the changing needs and aspirations of users.
- Ensure compliance with Supporting Young People standards and
Organisational policies and procedures.
- Organise and provide appropriate support for tenants preparing to move
- on to greater independence, including support through the move and resettlement into their new home.
- Liaise with other housing, health and social care professionals, benefits, welfare and advocacy agencies, and other landlords as required.
- Work towards operational and service delivery key performance indicators and to be proactive and efficient in data entry.
