We are looking for two extraordinary individuals to join our outstanding team of Service Coordinators, leading our supported living teams to provide excellent care and support to vulnerable adults with learning disabilities. Demonstrating a truly person-centred approach and the desire to empower others, you will coordinate services for people to maximize their independence, whilst working with your Service Manager to ensure services are run to a high standard in quality and safety, and within budget.
You will be responsible for leading, managing and inspiring Personal Assistants, who support the people living at services, through ensuring all support plans, risk assessments and records are regularly reviewed and relevant, as well as leading team and 1:1 meetings with your staff.
**THE ROLE**:
All our supported living services support people living in their own home, and/or with their own tenancies some of which are managed by Milestones Trust, or another housing association who we work in partnership with.
Our current vacancies are for two full-time Service Coordinators supporting teams working in a mix of 24-hour shared and 1:1 services in north Bristol, for people with learning disabilities, autism, or with an acquired brain injury.
Milestones Trust promotes person centred care and will always aim through the recruitment and interview process to match the right applicant to the right patch of services, and as such our service coordinators may be asked to move between different patches over time where their skills are recognised and required to support the needs of the business.
This is an exciting opportunity for someone with care management, team-leading or other leadership experience to start something new and have a positive impact on the lives of the people we support.
**The successful service coordinator will**:
- Be able to inspire and lead a team, demonstrating excellent people-management skills together with enthusiasm and dedication
- Be passionate about citizen rights, inclusion and community, with an understanding of the Mental Capacity Act, Safeguarding Adults, and other relevant legislation
- Be an excellent communicator, confident liaising with families, professionals and other agencies involved in peoples' lives
- Be proficient in IT and able to learn and use different software and systems commissioned by Milestones Trust for recording and communicating
- Be an excellent organiser, able to juggle competing priorities
- Be willing to perform occasional on-call duties as rostered with other service coordinator and manager colleagues, offering emergency phone support to services and teams outside office hours and over weekends
- Be creative and resourceful in your approach
