**Job Summary**

**The Role**

Come and join us as an Outreach recovery practitioner working in the borough of Kingston upon Thames. This is an exciting opportunity to support the homeless population into treatment to address drug and alcohol needs. Engaging with vulnerable individuals who are or may be at risk of homelessness and have experienced housing challenges, with substance misuse, alcohol, and mental health difficulties as part of a multi-disciplinary team. Making positive changes in peoples’ lives, through a non-judgmental, recovery-based approach that values the strengths and choices of those individuals.

**Role Purpose**
- To deliver excellent drug and alcohol street-based interventions to individuals who are rough sleeping, or at risk of homelessness and those in supported accommodation, to facilitate positive outcomes.
- To manage a complex case load and coordinate and deliver assessments and

evidence-based interventions (brief and structured), recovery planning and aftercare

planning.
- To work collaboratively with colleagues and key stakeholders, partners, across multidisciplinary teams, ensuring adherence with all Via policies and Safeguarding principles

The role will require dynamic partnership working; liaising with a broad range of services to provide wrap-around risk management focussed care. Assertively and creatively trying to engage individuals who may be treatment resistant or re-engage those that have had unsuccessful episodes of treatment in the past.

**The Service**

Via Kingston is a fully integrated substance misuse service, providing clinical and psychosocial interventions, operating from a main treatment hub in Surbiton. The team actively promotes recovery and works with individuals that are directly and indirectly affected by substance misuse, working with the local community to improve the health, wellbeing and functioning of Kingston residents, irrespective of circumstance.

**Location**

The role is based at the Via Kingston, but at times may involve travel to external meetings and satellites within the borough, and to other Via locations as needed.
