**Job Overview**:
As a Support Worker, you will support and advocate for individuals experiencing or facing homelessness. You will independently and working as a team, deliver tailored support to address their accommodation, health, and holistic needs. You will work closely with management, local services and partner agencies to ensure effective case management and service delivery.

**KEY AREAS OF RESPONSIBILITY**

**CASE WORK**:

- **Caseload**:Manage a flexible caseload supporting service users in an individualised approach, addressing accommodation needs, mental health, substance use, poor physical health, benefits and holistic needs. Caseload will be allocated by managers and will be vary depending upon projects and contractual commitments.
- **Complex Cases**:To support completing actions relating to identified complex cases, under the guidance of management. This will include but is not limited to - tenancy related issues, unmanaged mental and physical health, chaotic engagement with services.
- **Outreach**:Establish rapport with new or entrenched rough sleepers, facilitating referrals to temporary accommodation, assess support needs and develop action plans when required. This includes sharing information to partner agencies such as the local authority.
- **Accommodation Sustainment**: Assist service users in overcoming challenges to maintain both temporary and long-term housing. This involves coordinating with other relevant professionals and making necessary referrals.
- **Integration with Local Services**: Connect service users with relevant services (e.g substance use, health, criminal justice, DWP) and liaise with associated professionals. Ensuring accurate provision of sign posting information.
- **Progress Review**: Regularly review service user progress with management and partner agencies.
- **Intensive Support on SLA Projects**: Assist in providing intensive support to complex individuals on service level agreement projects to meet stakeholders’ performance targets and address support needs.
- **Crisis Management and Risks**: Support crisis presentations and emergencies, including providing immediate support and coordinating with relevant services to resolve the crises effectively. Support service users with identified risks and utilising safe working protocols.

**COMMUNITY LIAISON**:

- **Representation**: Act as a public representative of HTH, ensuring good community relations and professional conduct.
- **Partnership working**: Ensure accurate and prompt communication to relevant professionals to prevent duplication and coordinate support. Attend multi-agency meetings and sector events.

**ADMINSTRATION**:

- **Support Admin**: Maintain accurate records, action plans and completing referrals to other services. To update case management spreads and complete outreach coordination paperwork. Perform additional administrative tasks as requested.
- **Reporting**: Follow HTH policies and procedures relating to incidents and safeguarding concerns, including reports and referrals.
- **Database Management**: Input and extracting information from computerised packages and databases.

**POLICY AND TRAINING**:

- **Compliance**: Be fully conversant with all aspects of HTH’s policies and procedures and related protocols, e.g safeguarding, lone working, reportable incidents etc.
- **Training**: Undertake training identified by the management team and being proactive in identifying areas of personal development and performance progress.

**OTHER DUTIES**:

- **Meetings**: Attend internal and external meetings or HTH commitments, as directed by management.
- **Supportive Environment**: Maintain a friendly and supportive environment within the service.
- **Supporting service delivery**:Contribute to the day-to-day running of the service, including but not limited to - coverage due to staff absences and flexibility in assuming various responsibilities; maintaining presentable areas; assisting with unloading deliveries and setting up for the drop-in wellbeing hub.

The role may involve additional tasks or responsibilities not listed above, as required by the needs of the service and changes in commissioning. Management may assign other duties within the scope of the position’s skill level.
