We are seeking a reliable, enthusiastic person to join the Gastroenterology Team as a Patient Pathway Officer. We are looking for an excellent communicator, able to demonstrate and promote high levels of customer care.
Your role will be to deliver an efficient and effective appointment scheduling and outcome service to the Gastroenterology team, keeping up-to-date and accurate computer recorded information on the status of patients.
You will support the Senior Patient Pathway Co-Ordinator, Patient Pathway Co-Ordinator's, clinical team, and other divisional managers in ensuring the specialty are able to deliver safe and effective services whilst also ensuring compliance with contractual access targets e.g., 6-week diagnostic wait time.
Experience in using the following hospital administration systems is preferred but training can be given:
ACP
EPR
Clinic letters
Bighand
Your role will involve communicating rejection of inappropriate requests to requesting Consultants according to published guidance, monitoring demand and wait times, identifying potential breaches and escalate to Senior Patient Pathway Co-Ordinator and action appointments, entering details onto EPR as required.
You will ensure that patients have the correct verbal and written information regarding their tests and appointment date, advise patients within sphere of knowledge and according to protocol about the type of procedure, monitor and action DNA according to local and Trust protocol. Gastroenterology, Medicine Division and courteously and efficiently receive telephone enquiries from patients, carers and external agencies in relation to appointments ensuring that accurate records are maintained, and appropriate action is taken in a timely manner.
You will be responsible for negotiating dates with patients allowing for patient choice and assessing the viability of available slots in accordance with the Trust patient access policy and booking processes, for the arrangement of patient transport as necessary by liaising with the relevant hospital transport services and GP practices, ensuring that relevant documentation is completed, to ensure patient information is maintained and updated on the appropriate patient information systems and to maintain an accurate and up to date filing system of patient related documentation.
