Our award winning Cardiothoracic Centre (CTC) has a fantastic opportunity for you to join its close knit, friendly team as our Patient Pathway Coordinator.

In your new role you will be required to work within a specified area to facilitate the smooth running of the patients journey from referral to treatment, providing admin support to all clinical staff including medical, nursing, managerial and administration teams.

You will monitor and maintain clinic utilisation working within the Referral to Treatment (RTT) 18 week pathway, monitoring the patients journey from referral to treatment to ensure smooth with the ability to support and maintain safe and effective services whilst compliance to meet Trust Key Performance Indicators (KPI)

You will also be expected to provide cross cover for colleagues to ensure a consistent and effective administration service is maintained at all times. and to attend and record minutes at various meetings as and when required to do so.

Your role involves coordinating, monitoring and initiating appropriate changes within administrative processes along the patient pathway to ensure the patient's journey is managed efficiently, smoothly and in accordance with targets and agreed timescales.

You will utilise computerised tracking systems to locate case notes and investigation results, liaise with teams outside of the specialty to ensure smooth patient journey and ensure patient information is maintained and updated on the appropriate patient information system.

You will also monitor work lists and Consultants workflow, escalate to operational teams as and when required to do so.
