An exciting opportunity has arisen to appoint an 18 month fixed term/secondment, full time, band 3 Family Support Worker in our established Keyworker Team, based at Dragon Square, Chesterton.

We are looking for someone to work within our keyworker team to offer the step-down model for clients who sit within a dynamic support register. The postholder will draw upon personal or learned experience when working with clients with learning disabilities and/or autism and allocated to them from the keyworker team. This role will focus on advocacy and bringing the voice of the family and client to all case discussions.

Family support workers experiences can come from a variety of lived or learned experience:

- Direct experience of caring or living with a child or young person who has either a learning disability, autism or both
- Experience of being a Parent or carer for someone with a learning disability, autism or both
- Experience of working to support people who present behavioural challenges in the community

- To display a genuine desire and/or experience in supporting LD/A CYP and their families to access the right support at the right time, through relevant escalation within the team
- Training (or the ability to engage in training) to support families emotionally and practically through helping them access resources in the community to support themselves and their CYP
- A good understanding of voluntary and third sector organisations that would assist with early discharge of these clients - being supported in the community
- Support keyworkers through their role engaging and including parents as equal players in planning care and through considering options and choices in the treatment of their CYP
- Attendance at regular meetings-virtually and/or in person and at varied times as required in order to enable feedback on key issues to both professionals and parents
- A proactive plan of training to support your own development
- To listen to Parents and CYP’s concerns and issues, and escalating these where appropriate, ensuring your Team Lead is aware of important issues, with their consent.
- To provide relevant information to support Keyworker attendance at professional network meetings
- Where these exist to network with other family support workers within the team, sharing best practice; supporting and covering each other as required
- To keep good records of meetings, Lorenzo notes and submitting reports in a timely manner with key issues raised
- To participate in all relevant training that supports the role to deliver for LD/A families.
- To attend supervision meetings held by the keyworker team lead or keyworker Band 6’s
- To alert keyworker team lead on themes or challenges being experienced as adverse to LD/A children and Families
- To be flexible to offer out of hours (by appointment), only agreed through Team Lead
