**About Directorate**

You will be working within the Adults & Health directorate. The role will sit within the Shared Lives Service

**About the role**
- To work in a registered Shared Lives Scheme in partnership with Shared lives Carers to ensure the needs of vulnerable people using the Scheme are met.
- To respond to referrals by matching and placing people with Shared Lives carers
- To support the Registered Manager with the day to day running of the scheme and to assist with completing various tasks in the absence of the Registered Manager and to ensure the Head of Service is kept informed of any issues as they arise
- To provide support in the recruitment of families and individuals to provide day support, respite and full time placements on a short or long term basis to adults with learning disabilities
- To maintain a consistently high standard of service to service users by monitoring the standards of service provided, giving timely, constructive feedback to address performance gaps.
- To work within the appropriate policies, procedures, statutory requirements and national minimum standards, and complete associated paperwork, to promote and deliver outcome focused, choice based, person centred and evidence based social care practice

**About you**
- QCF Level 3 or equivalent
- Experience in the social care field, nursing or teaching fields
- Experience of working in a multidisciplinary environment with conflicting demands and priorities
- Ability to work within a legislative framework and to follow policies and procedures
- Demonstrate an understanding of, and commitment to, equal opportunities
- Knowledge and understanding of the Shared Lives model
- Demonstrable experience in successfully engaging with a wide range of stakeholders, including less heard groups
