An exciting opportunity has arisen within the Quality & Safety Directorate to recruit to the post of Serious Incident (SI) Investigator.

You will be part of the Safer Care Team; who will also work in close collaboration with the Head of Safer Care & Standards, Managing Directors, Service Line Leads and the Safeguarding Team to ensure a coordinated approach to safeguarding investigations and Operational engagement in the investigation process.

You will be expected to undertake investigations on behalf of the Trust and will be competent in utilising root cause analysis and human factors methodology and producing high quality, evidence based reports.

You will be required to provide an objective assessment of the events that led to an incident occurring, to identify areas of learning directly related to the findings and demonstrate compliance with the NHS Duty of Candour by engaging service users and families in the review process.

Support of staff post incident and facilitation of reflective learning sessions will be a vital part of the role.

Must have current professional registration as a Registered Mental Health Nurse, Social Worker, Occupational Therapist or equivalent.
