Pre-operative Assessment Pathway Coordinator

An exciting opportunity has arisen within our centralised Pre-Operative Assessment (POA) Service on the Denmark Hill site. As part of the expansion and development of the Pre-Assessment, this is a newly created post owing to the needed monitoring and support of the electronic Pre-Assessment Remote Arm Questionnaire. The successful post holder will be required to work as part of a multi-disciplinary team with POA Team, surgical specialities and the Day Surgery Unit.
They will be able work well in a team, against tight deadlines and have brilliant communication skills.

- Co-ordinate and provide administrative support to the relevant teams to ensure that all relevant paperwork and clinical information is accessible.
- Act as a central point of contact for communication with the clinical teams and patients.
- Ensure confidential and sensitive information in relating to patients is kept secure and follows the policies and procedures for the department and the Trust
- Expected to proactively assist in maintaining high standards and professional practice.

JOB SUMMARY
The Pre-Operative Assessment Pathway Co-ordinator will monitor and lead on the administrative duties of the Pre-Operative Assessment Screening Programme for the Centralised Surgical Pre-assessment service at Denmark Hill. This includes providing the first point of contact for all Remote Arm, PIMS and Envoy Messenger related issues.
The post holder will be expected to prioritise conflicting work demands and effective working with the Pre-Assessment and Day surgery clinical teams, the multi-disciplinary team, internal departments and external agencies to ensure continuity in achieving excellence in patient care.
MAIN DUTIES AND RESPONSIBILITIES
Remote Clinic Co-ordination
- Distribute electronic Patient screening questionnaires and administer in a line with deadlines and requirements as set out in Trust Polices.
- Implement and maintain an efficient internal system and process to ensure the pre-operative pathway is followed through.
- Monitor and review patients Pre-operative assessment progress using the Patient Tracking list (POA PTL Dashboard) in line with Trust and local polices.
- Ensure all services are updated on the outcome of the screening clinic and informed of direct patient queries.
- Update on all enquiries regarding patients’ care and/or treatment on a pathway from external Trusts and General Practitioners in a timely manner.
- Ensure that all data is inputted and/or uploaded accurately into the computerised hospital administration system administration system (PIMS and Remote Arm) in a timely manner.
- Work within a multi-disciplinary team in developing the service in line with departmental plans and Trust corporate objectives.
- Ensure all patients receive electronic or paper patient information leaflet relevant to their procedure.
