Outpatient Services Pathway Coordinator- Royal

Outpatient Pathway Coordinators will be responsible for the operational support across these departments ensuring that the patient pathway through the outpatient service is streamlined administratively and unhindered to enable referral to treatment (RTT) targets to be achieved.
The Outpatient Pathway Coordinator will be for responsible for ensuring that data quality standards are met as part of referral management and for every outpatient episode in all Trust systems.
The post holder will be required to promote and adhere to best practice in accordance with the Trust’s Patient Access Policy and utilise all relevant Trust Policies related to the fulfilment of the Outpatient Pathway Coordinator post to ensure competency in all aspects of the role.
To be responsible for management of the patient pathway from the initial appointment, to ensuring that each patient has the relevant follow-up care booked in coordination with pre-booked tests within internal milestones to achieve national targets
To be responsible for supporting the planning and preparing of clinics in advance to ensure the efficient running daily and deliver excellent patient experience.
To ensure that patients are provided the earliest reasonable offer date providing 3 clear weeks’ notice of their appointment and tests (imaging, exercise test etc) are coordinated ahead of their consultation.
To ensure that patients are communicated with after a did not attend (DNA) to ascertain reasons to support the clinical decision to complete routine discharge or rebook clinically urgent pathways.
To be a point of contact for patients, carers, relatives, and other health care professionals where pathway concerns are raised and require resolution.
To provide a friendly, informative service to all patients and their relatives ensuring pathways are not
adversely affected and providing resolution where challenges arise.
To support data quality audits and analyse adherence to local SLA’s and key.
The post holder will work with the Outpatient Administrative Coordinator in all aspects of improving outpatient flows and pathways.

To be responsible for ensuring that electronic outcome lists have been completed by clinical teams and are processed for the entire patient list at the end of the clinic session in Trust systems Ensuring all necessary actions having been taken (e.g. procedures coded, follow-ups and diagnostics coordinated and pre-booked, TCI forms with priority level processed).
To supervise the Outpatient Administrators and ensure that the designated areas have sufficient staff resource to maintain delivery of an excellent patient experience particularly during peak service periods and staff annual leave.
Communicate with staff and external agencies professionally and sympathetically always maintaining patient confidentiality when.
Communicate clinic session changes to all outpatient teams when accommodating clinical leave (annual/study other) and providing outcomes to Outpatient Administrative Coordinators.
To work closely with the other Outpatient Administrative Coordinators to ensure shared working and cross cover during periods of annual leave
Support the personal and professional development of staff through the Trust’s Performance Management and Staff Development Programme, ensuring the education, training, and development of staff as appropriate.
Maintenance of computerised Patient Administration System by inputting up to date information received from patients or other NHS sources.
Maintain Trust scheduling system for all patients who have arrived, seen, did not attend, discharged, or departed their clinic appointment and ensuring all appointment information is always up to date
