An exciting opportunity has arisen for Administrators to work within the Somerset Hub for Coordinating Care team.

The hub consists of the Intermediate Care Bed Hub, Somerset Primary Link GP Referral Service, Hospital at Home Remote Monitoring Service and End of Life Care Coordination Centre.

This job role will predominantly be based within the Somerset Primary Link but successful applicants will have the opportunity to work across the teams.

You'll have strong attention to detail and will be comfortable working within a busy working environment.

Our service hours are 08:00-21:30 7 days a week 365 days a year and will be worked in shift patterns, this includes bank holidays.

**Main duties of the job**

You'll work closely with the Somerset Hub for Coordinating Care clinicians in facilitating a smooth referral process for service users and patients, providing a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust.

You'll also support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process - to include booking transport and liaising with patients.
