An exciting opportunity has arisen for Administrators to work within the Somerset Hub for Coordinating Care team.
The hub consists of the Intermediate Care Bed Hub and the Somerset Primary Link GP Referral Service
This job role will predominantly be based within the Intermediate Care Bed Hub but successful applicants will have the opportunity to work across the teams.
You'll have strong attention to detail and will be comfortable working within a busy working environment.
Our service hours are 08:00-21:30 7 days a week 365 days a year and will be worked in shift patterns, this includes bank holidays.
You will work closely with the Somerset Hub for Coordinating Care clinicians in facilitating a smooth referral process for service users and patients, providing a single point of contact for information on patients to be transferred / admitted to community hospital beds within the Primary Care Trust.
You will support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process - to include booking transport and liaising with patients.

- To participate in a multidisciplinary team working toward a whole system approach to Trust wide bed management system.
- To co-ordinate and support exchange of information concerning a patient’s care between hospitals, GPs and community professionals.
- To develop, maintain and manage the Bed Management Databases so that an accurate and detailed list of patients with suitability for transfer can be presented and updated daily.
- Support the Somerset Hub for Coordinating Care clinicians in providing an efficient referral process - to include booking transport and liaising with patients.
- To develop, maintain and improve systems, which provide details of the patient fitness status for patients waiting for transfer.
- To develop, maintain and provide accurate and up-to-date Trust wide information on delayed transfers of care for reporting weekly at regional level.
- In consultation with Senior Nurses, prioritise use of community hospitals beds.
- To work within the Trust guidelines and Policies for admission and transfer of patients.
- Facilitate the working of project teams relating to the introduction and maintenance of booked admissions.
- Facilitate the involvement of all relevant staff and patients with the programme and its projects.
- To work within the Somerset Hub for Coordinating Care service to ensure that waiting lists are managed and maintained according to the Trust’s policy and actively manage the patient journey to appropriate beds once patients become medically fit for transfer.
- To liaise with and support staff involved in managing waiting lists.
- Establish and maintain the necessary administrative procedures to ensure smooth teamwork.
- To provide support to the Somerset Hub for Coordinating Care clinicians as appropriate.
- Ensure effective operation of Trust and professional policies and procedures.
- To co-ordinate and support exchange of information concerning a patient’s care between hospitals, GPs and community professionals.
- Record, evaluate and organise information for clinical and managerial action in Somerset Hub for Coordinating Care.
- Add case notes onto patient’s Rio notes when referred through Somerset Hub for Coordinating Care.
