This is an exciting opportunity to join our well established team working within the Intermediate Care model which is based in Wells. We are looking for a proactive Band 6 Registered Nurse who can support the Somerset Hub for Coordinating Care that delivers across Somerset.

The Somerset Hub for Coordinating Care operates 7 days a week 08:00-20:00 (including Bank Holidays) on a shift pattern.

This post will be utilised across all services on the second floor at Priory House consisting of Somerset Primary Link and the Bed Hub. This vacancy is 22.5 hours per week. The post holder will be primarily based at Priory House in Wells, when working in the Bed Hub you will also be required to work from Musgrove Park Hospital and Yeovil District Hospital on a regular basis. There may also be requests to join our colleagues at Weston and Bath at times to support flow therefore car driver essential.

- To establish and maintain a sound mechanism for transfer/discharge of patients in both primary and secondary care for Somerset NHS Foundation Trust.
- To work collaboratively across primary and secondary care to reduce delayed discharges.
- To work autonomously on a day to day basis, prioritising workload to provide cover between the Somerset localities within the Trust.
- To work in the Somerset Hub for Coordinating Carein the referral and sign posting of urgent care admissions by enabling the provision of care from a variety of agencies. The post holder will provide central liaison between the services which provide Rapid Response Rehabilitation and Social Care and the clinicians who refer patients.
- The post holder will clarify and action urgent referrals and will work with a wide range of multi-disciplinary and multiagency teams.
- The post holder will work as Clinical Navigator, working closely with call handlers to identify and assess patients facilitating the patient pathway through primary community and secondary care.
- To contribute and support Bed Management in the Acute Trust sites and liaising with clinical management site teams and the Discharge Liaison Team.
