**Job Role**
The community liaison lead will be responsible for ensuring all current and newly referred individuals receive high quality, person centred support.
You will support practice development within the team and will provide coaching and mentoring to team members. The community liaison lead role will include direct support and care provision.
You will carry out reviews and community observations of the staff team, ensuring quality of practice within the care team.
You will ensure good joint working practices are established and maintained.
You must be a driver with access to your own vehicle.
NVQ Level 2 preferred but not essential.
Potential applicants must be flexible.
PLEASE NOTE THIS ROLE IS ALTERNATE WEEKEND WORKING.
**Duties and Responsibilities**
- Ensure that all care plans are completed to a high standard
- Carrying out audits
- Carryout all administrative duties for effective care
- Complete accurate records and ensure that each individual has an up to documents
- Promote safe working practices and respond to emergencies as required
- Carry out quality checks
- To be part of the on-call rota system providing absence care cover
- Any other duties as and when required by the Directors.
**Staff**
- Provide coaching and mentoring to staff
- Be a visible and competent role model to the team
- Be available to work flexibly
**General**
- Adhere to all policies and procedures
- Ensure quality standards are maintained
- Attend meetings when required
- Attend and undertake learning and development as required
- Promote and ensure the good reputation of the organisation
- Carry out any other tasks reasonably assigned to you
**Hours / Environment**
Office based duties are carried out between 7-3 however you will be required to work shifts as part of a team providing 24-hour cover, seven days a week and have on-call duties on a rota basis.

**Salary**: £10.50 per hour
