We are looking for an enthusiastic individual to join our community stroke team as a Technical Instructor. This is varied role working as part of the MDT, alongside Occupational Therapists, Physiotherapists, Speech and Language Therapist and our Community Stroke Nurse. You would be implementing rehabilitation programmes in-conjunction with the other members of the MDT with the aim of improving client's function and well-being in the community to support their stroke rehabilitation following hospital discharge.

The role may require weekend and bank holiday working. As community visits will be required you will need to havefull valid UK driving license with access to a vehicle for work purposes.
1. To work with the therapists, in the implementation and progression of rehabilitation programmes for Physiotherapy, Occupational Therapy and Speech and Language Therapy.
2. To provide support to the community Stroke Nurse.
3. To provide a high standard of therapy assistant service to community based patients.
4. To facilitate patients to reach their optimum level of independence in mobility and activities of daily living.
5. The post holder will participate as a member of the multi-disciplinary team assisting with specified duties and to take responsibility for planning own work duties.
6. Perform non-clinical duties such as clerical or housekeeping which aids the efficient running of the service
7. The role is mainly Community based but there may times when you work in to the hospital as part of supporting a patient's discharge.
8. The role is predominantly Monday to Friday at the present time but some weekend and bank holiday working may be required.

Based in the North East of England we provide a range of hospital and community health services from our leading facilities including the Queen Elizabeth Hospital, Blaydon urgent care centre and Bensham Hospital, all within Gateshead.

Established in 2005, we were one of the first foundation trusts in the country and since then have consistently achieved the highest levels of care for patients.

As a group we employ around 4,200 staff and currently provide 600 hospital beds across Gateshead. Our primary focus is on providing excellent general health services to our local community complemented by key specialist areas including Gynaecology, maternity, palliative care and old age psychiatry.

Alongside a full range of local hospital services, we provide breast screening services for Gateshead, South Tyneside, Sunderland and parts of Durham - and we are the North Eastern hub for the National Bowel Cancer and AAA Screening Programmes, covering a population of around seven million people.
1. To work primarily with the Community Stroke Team.
2. To work as part of the multi-disciplinary team to deliver a holistic patient-centered approach to care.
3. To be responsible for implementing prescribed treatment programmes with an allocated caseload, following an initial assessment by a qualified Occupational Therapist/Physiotherapist/Speech and Language Therapist.
4. To work with therapists, planning and facilitating the patient’s progression through the treatment plan, within the protocols/plans set out by the qualified therapists.
5. To take responsibility for a delegated caseload and report outcomes appropriately to the overseeing therapist and wider multi-disciplinary team.
6. To undertake therapy intervention and treatment of a designated caseload following initial assessment by a qualified therapist looking at activities of daily living e.g. personal care, meal preparation etc; mobility and exercise provision seeking guidance from senior staff as appropriate.
7. To gather information about the patient’s home environment and under the advice of a qualified therapist provide appropriate equipment to suit the patient’s needs.
8. To be responsible for prioritising and planning of own daily caseload.
9. To document interventions accurately and promptly.
10. To follow lone working procedures.
