Colorectal Stratified Follow-up Co-ordinator and Admin Support

The Colorectal Cancer Service KCHFT is provided at both the Denmark Hill and Orpington South Sites which covers a large geographic area covering South East London boroughs of Southwark, Lambeth and Lewisham.
The Colorectal Team at the Princess Royal University Hospital is situated on the Surgical Floor (Level 2, North Wing). The Colorectal Service serves the population of Bromley, which is the largest borough of London. The Colorectal Multidisciplinary Team (MDT) at the PRUH site serves the 300,000+ population of the London Borough of Bromley. In addition, referrals are commonly received from Bexley, Sidcup, Sevenoaks, and Westerham.
Both units combined diagnose in excess of 400 new colorectal (bowel) cancer patients each year. As a part of the National Cancer Strategy, a new pathway for bowel cancer patients has been developed and rolled out across London and England.
Stratified follow-up is a model of aftercare that addresses the unique needs of the individual living after a cancer diagnosis and also empowers individuals to self-manage their health. Implementation of stratified follow-up with a focus on self-management reduces the frequency of hospital based follow-up appointments, and ensures that patients are at the centre of their self-management, by steering individuals onto the best pathway to meet their specific needs.
Help coordinate the patients who are going on to stratified follow up for LGI Cancer.
Book all patients appropriately on to the specialist Remote Monitoring system.
Track all bowel cancer patients who are on stratified follow up across Kings and PRUH – ensuring Colonoscopy and CT scans are being managed and reviewed at appropriate intervals, and to raise cancer follow-up surveillance results to clinicians so a correspondence can be sent to the patient and their GP in a timely manner.
Provide support to the Colorectal CNSs with all elements of the recovery package
This post is designed to act as the key support for the Colorectal CNS's; organising and supporting their team meetings, tracking patients pathway and ensuring timely and accurate collection of data. The post holder will also be required to produce reports and undertake audits as required.
This is a role with lots of variety and ad hoc demands and the post-holder must possess excellent time management and planning skills. This post may develop over time to meet the specific needs of the clinical unit and the changing demands on the service.
Liaise with departments involved in providing care and core services to ensure that Survivorship Targets are met and highlight to the recovery package project manager when we are behind trajectory so that corrective action can be taken.

The post holder will have specific responsibility for helping to embed and then for maintaining robust systems for collecting and analysing clinical information on bowel cancer patients. This will involve being familiar with the trust and cancer information systems, and being able to enter, extract and manipulate data.
The post holder is the principal member of staff responsible for managing the enrolment and digital safety-netting of patients who have entered a period of surveillance following completion of treatment for colorectal cancers who are eligible for Personalised Stratified Follow-up.
