Our Macmillan Support Workers work as part of the cancer care team, alongside registered practitioners to improve care for people with cancer. The Support Workers provide a point of contact for patients and people affected by cancer that helps them navigate through the care system and to live as well as possible. The Support Worker is a valued member of the cancer care team, freeing the time of registered practitioners to focus their expertise on managing the most complex care needs. Support Workers provide coordination of care to people mostly after cancer treatment, enabling them to self-manage their own care with support. The Support Worker will be one of the primary points of contact for our patients, they will offer holistic needs assessments and on-going support; they will triage urgent calls and can attend clinics. A large part of the role involves assisting the Clinical Nurse Specialist team in delivering complex care and support and developing pathways to improve the patient experience. The Support Workers will take responsibility for organizing and delivering health and wellbeing events and contribute to peoples ability to live well after they finish their treatment. All posts include some patient tracking and data collection and therefore requires competent use of Microsoft packages, including Excel and preparing information for presentation. The scope of the role will vary between settings however the main purpose of the Macmillan Cancer Support Worker is to: 1. Provide a single point of access for cancer patients with the most common solid tumour types (breast, colorectal and prostate cancer); 2. Provide appropriate advice and escalate to a registered practitioner (Cancer Clinical Nurse Specialist) where required; 3. Co-ordinate access to the right information and education resources to support people affected by cancer in making decisions; 4. Offer key Recovery Package elements to cancer patients including Holistic Needs Assessments, Treatment Summaries and Health and Wellbeing Events. 5. Navigate breast, prostate & colorectal cancer patients who have completed their treatments through stratified follow-up pathways, including self-management, for the duration of their post-treatment surveillance which may last up to 5 years.
