Job Description
The Case Manager role is to effectively support our clients to ensure high quality, consumer directed care. The role requires proactive case management and customer service across various funding streams.

Duties Of The Role Include

  • Provide information and guidance on services to our clients
  • Complete intake visits and create care plans
  • Conduct initial and ongoing assessments that meet client needs, goals and preferences
  • Ensure services are delivered with dignity and respect in a safe environment
  • Respond and communicate with clients re changing care needs
  • Facilitate support services from external providers
  • Act as an ambassador for the organisation through attendance at community events
  • Maintain and ensure currency of electronic records and case notes

Desired Skills And Experience
The successful candidate will need:

  • Proven experience in Case Management in NDIS or Aged Care
  • Intermediate skills in Word, Excel and Microsoft Office
  • Experience working with service related data bases, Alayacare experience desirable
  • Current Driver's License, own registered and fully insured vehicle
  • Satisfactory National Police Check
  • NDIS Worker Screening check
  • Passion and commitment to delivering exceptional consumer directed care
  • To be able to work autonomously with a supportive and cohesive team
  • To be reliable and committed to promoting organisational growth
  • To be able to prioritise, manage time and deliver services within agreed time lines
  • Good oral and written communication skills
  • To be highly motivated, organised, proactive and efficient
