Senior Clinical Practitioner - Diabetes / Vascular
Full-time and part-time Senior Clinical Practitioner £48,450 to £53,818 depending on experience/ KSF mapping.
gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England.
We have fantastic opportunities for Senior Clinical Practitioners to support work within a community-based meeting of Multidisciplinary Team (MDT) clinic across the Oldham area on a 12 months fixed term contract.
As a Senior Clinical Practitioner, you will work within a clinic for patients referred from their GP due suspected peripheral arterial disease (PAD). Their journey will require a comprehensive vascular assessment, diagnosis (including ABPI) and a management plan, which may include a diabetic review +/- medication changes, lifestyle advise / health promotion and onward referrals where appropriate.
The successful candidate will be a skilled Senior Clinical Practitioner with expertise in either diabetes and / or vascular, you will be passionate about diabetes care and wants to make a real difference in the lives of patients managing diabetes a nurse led- primary care service.
You will have a specialist interest in diabetes management and / or experience of the assessment and management of PAD. This is an exciting opportunity to be part of a dedicated team and help deliver high-quality, patient-centred diabetes care within a community setting.
As an Senior Clinical Practitioner, the main duties of your job will include:
- Comprehensive assessment and diagnosis of PAD.
- Providing holistic care to patients with diabetes, including monitoring, education, and support.
- Leading and supporting the MDT clinics and initiatives, collaborating with GPs, multidisciplinary teams and local services to optimise patient outcomes.
- Promoting best practice in line with current guidelines and providing data on patient outcomes.
- Assess, order and interpret investigations, undertake practical procedures (including APBI), diagnose, plan, implement and evaluate treatment / interventions and care for patients presenting with symptoms of peripheral arterial / vascular disease.
- Clinically examine and assess patient needs from a physiological, psychological and social perspective, and plan clinical care accordingly.
- Diagnose and manage patient referred into the well-leg service, integrating both drug- and non-drug-based treatment methods into a management plan, including insulin initiation.
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Proactively support the assessment of frail elderly patients or those with complex healthcare needs, working with admission avoidance pathways/services to enhance care.
