Overview
We are seeking a well organised Care Coordinator to join our team at Hapus Home care.
The ideal candidate will play a crucial role in ensuring that our clients receive the highest standard of care and support. This position involves coordinating various aspects of client care, staff rotas, liaising with healthcare professionals, and providing essential administrative support. The Care Coordinator will be pivotal in enhancing the overall experience for clients and their families.
This role initially is to be worked over two set office days a week, preferably a Monday & Tuesday, between 9am and 3pm, and will include covering on call on a Rota basis each week with the deputy manager.
There is an option for extra hours working on the community, as it is very important for staff and clients to be well supported.
You do need to be a car driver due to the role and responsibilities included in the coordinator role.
The coordinator role also requires some flexibility to be able to step in as early as 7am at short notice if required, and to cover on call on a rota basis working with the deputy care manager.
You will receive additional enhanced pay for this on top of the hourly rate you receive for the two set coordinator office days.
Responsibilities
- Coordinate and manage client care plans, ensuring that all services are delivered effectively and on time.
- Liaise with healthcare providers, social workers, and other professionals to facilitate communication regarding client needs.
- Maintain accurate and up-to-date client records, including documentation of services provided and any changes in care requirements.
- Assist clients and their families in navigating the healthcare system, providing information about available resources and services.
- Carry out care and support visits when the need arises.
- Organise appointments, transportation, and other logistical arrangements for clients as needed.
- Support the administrative functions of the office, including managing correspondence, scheduling staff and client Rota's, while maintaining strong, supportive relationships with clients and staff.
- Be well organised in problem solving, with a willingness to stand in for others when required, to meet the needs of the business.
Skills
- Strong organisational skills with the ability to manage multiple tasks simultaneously.
- Excellent communication skills, both verbal and written, to interact effectively with clients, families, and healthcare professionals.
- Proficient in office software applications (e.g., Microsoft Office Suite) for documentation and reporting purposes.
- Empathy and compassion towards clients' needs, demonstrating a client-centred approach to care coordination.
- Ability to work independently as well as collaboratively within a team environment.
- Attention to detail is essential for maintaining accurate records and ensuring compliance with care standards.
