Job Introduction
As a Band 4 practitioner within our Community Rapid Response Team, you'll support the delivery of timely, effective rehabilitation and short-term interventions for adults in their own homes.

Your role directly contributes to reducing unnecessary hospital admissions and enabling a safe, supported discharge. You'll be working both autonomously and within a multidisciplinary team to carry out delegated assessments, deliver goal-focused interventions, and monitor patient progress.

You'll be equipped with a tablet device to work efficiently in the community, ensuring you can deliver responsive, patient-centred care wherever it's needed.

Main Responsibility
As a Band 4 Rapid Response Assistant Practitioner, you will:

  • Work proactively as part of the multidisciplinary team (MDT)—including nurses, occupational therapists, physiotherapists, GPs and acute hospital colleagues—to deliver high-quality community-based care.
  • Carry out delegated assessments, including basic functional assessments and observation-based clinical information gathering, feeding back findings to registered clinicians.
  • Contribute to the planning, delivery and review of individual rehabilitation programmes under the guidance of registered professionals.
  • Provide short-term rapid response interventions for patients referred via the Discharge to Assess (D2A) pathway and the 2-hour community response standard.
  • Support the completion of the Trusted Assessor process (and undertake full training when available), enabling timely and safe provision of equipment and adaptations.
  • Deliver and promote a high standard of rehabilitation, focusing on mobility, safety, confidence and independence with daily living tasks.
  • Work autonomously within the scope of the role, prioritising workload, recognising changes in patient presentation, and escalating concerns appropriately.
  • Use initiative to contribute to the smooth running of the service — including administrative tasks, equipment checks, communication with families, and supporting team organisation.
  • Maintain accurate clinical records using digital systems and contribute to MDT discussions and decision-making.

The Ideal Candidate
We're looking for someone who brings:

  • An NVQ Level 3 in Health and Social Care (or equivalent) PLUS relevant experience, with a willingness to complete Level 4 modules or Assistant Practitioner development where required.
  • Strong communication, problem-solving and organisational skills.
  • Experience delivering clinical or therapeutic interventions in a community, acute, or rehabilitation setting.
  • Confidence working both independently and as part of a multidisciplinary team.
  • A flexible, proactive approach, ensuring every patient receives a safe, positive experience.
  • A full driving licence and access to a car, enabling travel across the community.
  • Confidence using digital systems, including Microsoft Office and clinical records platforms.
