Use your advanced skills in long‑term condition management to assess patients at home, prevent hospital admissions, and deliver true holistic care when it’s needed most We are seeking enthusiastic Paramedics or Specialist Nurse to lead our established Early Intervention Vehicle (EIV) Service. Working within the Unplanned Care Team, your primary goal is Crisis Management and Admission Avoidance. You will be the clinical lead on the ground, responding to non‑life‑threatening priority calls across the Herts Valleys. You will work side‑by‑side with a Therapist, Social Worker, or CCO. Together, you will perform holistic physical and functional assessments, issue urgent equipment, and put immediate care plans in place to keep the patient safe at home. Main duties of the job Why this role is different: True Multidisciplinary Working: You aren’t working solo. You will assess jointly with therapists to tackle both the medical and functional crisis. Advanced Clinical Decision Making: You will use your advanced skills to make complex clinical decisions quickly and autonomously. You will have the discretion to manage risk and support patients in crisis. Direct Impact: You will directly reduce unnecessary hospital admissions and facilitate early discharges (Discharge to Assess). We are looking for HCPC registered Paramedics or UK NMC registered Specialist Nurse with: Advanced clinical assessment skills. The confidence to make autonomous decisions in a community setting. Experience in urgent care, community care, or the management of long‑term conditions. A driving ambition to develop the service and strategic skills. This is a dynamic, mobile role covering all 4 Herts Valleys localities. You will respond rapidly to patients’ residences, providing emergency equipment and clinical interventions. Shifts are currently 08:00 - 20:00, offering a structured roster pattern. Applicants MUST hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010). Person Specification Qualifications Registered Nurse with current NMC registration OR Paramedic with HCPC registration Evidence of recent and continuing post‑registration professional education and training equivalent to post‑graduate diploma level Experience of working in Primary Care/ Accident & Emergency Experience of assessing and providing quality care to people with Long Term Conditions Qualifications in Community and/ or Accident and Emergency health care Qualification in teaching or practice based learning Physical Clinical Assessment Skills Course or willingness to undertake the course Experience Evidence of recent substantial post‑registration professional clinical experience Extensive range of clinical skills and experience including, e.g. IV therapy, venepuncture, cannulation, rehabilitation Experience of working within a multi-disciplinary team Experience of supervising junior staff and co‑ordinating a team on a day to day, regular basis Experience of undertaking comprehensive health needs assessment and writing care plans Experience of implementing evidence‑based care in practice Skills and Knowledge Has high level of competence in a range of clinical skills Ability to work in a team Able to work without direct supervision, planning and prioritising own workload and that of others Able to delegate work effectively and safely Ability to work under pressure Ability to risk assess a situation and act appropriately Ability to communicate clearly and effectively, in both verbal and written format Ability to identify own and others learning and development needs and actively seek ways to meet those needs Basic IT skills including use of Microsoft Office packages and email Principles of care for patients with long term conditions, relevant NSFs and other national standards NMC/HCPC Codes of Professional Conduct and application to practice Personal Development Planning Primary Health Care - structures and roles (overview) The audit process Applying evidence based care into practice The principles of clinical governance and application to practice Clinical supervision - purpose and application Managing difficult situations and conflict resolution How equal opportunities can be implemented in practice Able to lead and motivate a team Other Hold a full, valid, UK driving licence and have access to a car to use for business purposes (unless you have a disability as defined by the Equality Act 2010) Able to carry out the duties of the post with or without adaptions Able to travel around the division Able to work flexible shifts Able to provide evidence of where you have demonstrated the Trust’s Values and Behaviours Able to meet the required IT Skills for the post Disclosure and Barring Service Check This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions. £40,617 to £48,778 a yearPer annum, inclusive of HCAS (pro rata) #J-18808-Ljbffr
