Job Overview CLCH Hertfordshire adult community services at CLCH NHS Trust are recruiting an experienced Paramedic or Specialist Nurse to lead our Early Intervention Vehicle (EIV) Service. This role offers independence to see and treat patients at home, prevent hospital admissions, and deliver holistic care when most needed. You will be the clinical lead on the ground, responding to non‑life‑threatening priority calls across the Herts Valleys and working alongside a therapist, social worker, or CCO to assess patients physically and functionally, issue urgent equipment, and implement immediate care plans. Key Responsibilities Assist in multidisciplinary working with therapists to address both medical and functional crisis. Use advanced clinical decision making to evaluate complex situations quickly and autonomously. Directly reduce unnecessary hospital admissions and facilitate early discharges (Discharge to Assess). Respond swiftly to patient call‑outs across all four Herts Valleys localities using a structured roster pattern (08:00‑20:00). Maintain a full, valid UK driving licence and access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010). Lead the establishment and development of the EIV Service, contributing to service design, strategy, and improvement. Collaborate with the Unplanned Care Team, Therapist, Social Worker or CCO to deliver comprehensive care plans. Job Details Shifts run from 08:00 to 20:00 on a structured roster pattern. Applicants must hold a full, valid UK driving licence and have access to a car for business purposes (unless you have a disability as defined by the Equality Act 2010). Qualifications HCPC registered Paramedic or UK NMC registered Specialist Nurse. Advanced clinical assessment skills. Confidence to make autonomous decisions in a community setting. Experience in urgent care, community care, or long‑term condition management. Driving ambition to develop the service and possess strategic skills.
