Primary Care Facilitator

As an expanding team, we have an exciting vacancy for a Primary Care Facilitator to work as part of the Primary Care Services Team within Digital Health and Care Wales. As a Primary Care Facilitator, you will be responsible for the delivery and support of digital technologies to Primary Care Stakeholders. We are looking for a motivated individual who is willing to work, learn, and actively contribute within a team environment, possessing excellent communication and organisation skills. Experience in the delivery of digital technologies is desirable but not essential. Due to the geography and spread of our service portfolio, you will be required to be flexible and be able to travel to various sites across Wales at least 3 times per week; this may on occasion, require overnight stays. This post is fixed term/secondment for 12 months to cover secondment. Main duties of the job The key duties of the role are: Supporting the implementation of national or local initiatives by delivering digital technologies to a broad range of Primary Care Stakeholders Coach and mentor Primary Care Stakeholders in adopting new digital technologies Build relationships with Primary Care Stakeholders to effectively manage their digital service needs Monitor service requests and act as an escalation point for Primary Care Stakeholder issues, liaising with DHCW, Health Boards and Suppliers, as necessary. Establish and maintain effective two way written and verbal communication with all Stakeholders Providing regular reports to Stakeholders on the progress and effectiveness of the IT services provided, ensuring compliance to the national standards and best practice is adopted Contribute to the development of project plans which identifies resources, critical paths, and risks. The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply. Job responsibilities Responsible for: adjusting priorities to meet frequently changing Stakeholder demands, self-managing any conflicting demands, providing coaching to Primary Care Stakeholders on the Digital products and services available to them maintaining effective two way written and verbal communication the implementation of national or local initiatives. providing feedback on new processes and procedures to support changes to service implementation, ensuring continuous service improvements to Primary Care Stakeholders. the preparation and drafting of routine and ad-hoc reports maximising the use of technology to improve performance and making recommendations to the teams operational processes to improve performance across the team. providing and receiving complex or sensitive information in relation to products/services projects and workstreams that will need to be interpreted and communicated to team members facilitating the deployment of national products and services through agreed communication channels Person Specification Qualifications Degree level or equivalent qualification or able to demonstrate equivalent experience Commitment to continuing professional development for self ITIL Foundation Certificate or demonstrable equivalent experience. Working knowledge of Clinical Terms. Knowledge of NHS Wales organisations & ITIL strategy. Knowledge of relevant NHS standards and legal requirements relating to Information Governance and Security Experience Experience of delivering customer services in a health care setting Experience in delivering projects in a Primary Care setting Presenting to small groups Skills and Attributes Ability to work on own initiative whilst being an effective member of a hybrid team Proactive & flexible approach to working Responsive to changing environments Excellent organisational skills and the ability to identify priorities Possess excellent written and oral communication skills. Ability to develop and maintain effective working relationship with colleagues and Stakeholders.
