Hours – Long days and nights.0715‑1900 days1900‑0715 nightsPotential weekend and bank holiday cover. Salary – This position is full time, you’ll receive an annual salary up to £40,211 per annum FTE – depending on experience. What you'll be doing Work within a multi-disciplinary team to deliver a nurse‑led service and prevent hospital admissions. Lead the development of individualized patient care plans, collaborating with the inpatient team and championing clinical excellence. Support patients daily to achieve activities of daily living, ensuring the best quality of life. Ensure quality care planning and continuity through improving relationships and communication with partners and other services. Deliver non‑judgemental care that makes a real difference, as defined through our bespoke Health in Justice competency framework.
