Job Summary Band 5 Nurse – 37.5 hours per week – Pershore Community Hospital, Herefordshire and Worcestershire Health and Care NHS Trust. We are recruiting a dedicated Band 5 nurse to support the inpatient ward of 26 beds, working closely with the multidisciplinary team (MDT) to achieve rehabilitation goals. Key Responsibilities Deliver person‑centred nursing care and support to ward patients. Administer medications and support the day‑to‑day running of the ward. Facilitate 24/7 rehabilitation to enable patients to reach the highest level of independence. Participate in MDT meetings, discharge planning and CHC reviews. Maintain accurate, contemporaneous patient records in line with Trust and NMC Code requirements. Assist with health‑promotion strategies and support health‑governance processes. Engage in research, audit and quality improvement projects. Deliver safe, effective and compassionate care with a focus on reducing length of stay. Required Skills & Abilities Strong communication and teamwork skills. Independently assess, plan, implement and evaluate nursing care. Ability to work without direct supervision and to handle pressure. Reflective practitioner with a positive attitude and ability to act on feedback. Excellent written and verbal communication in English. Basic IT proficiency. Compassion, empathy and patient advocacy. Qualifications and Experience Essential Registered General Nurse (RGN/RMN) with NMC registration. Experience gained during pre‑registration training. Ability to carry out physical requirements of the role, including mechanical hoists. Original certificates available for verification on offer. Desirable Practice Supervisor/Assessor training. Accreditation in Palliative Care, Tissue Viability, Diabetes or Dementia. Experience in frailty or learning‑disability settings. Advanced medication administration skills (PO, PR, S/C, IM, IV). Knowledge of Mental Capacity and DOLs. Experience of audit and research. Salary & Contract £31,049 to £37,796 per annum – Band 5 – Permanent, Full‑time (37.5 hours per week). Working Pattern & Schedule Rotational shifts. Not suitable for newly qualified nurses. Benefits 27 days leave plus bank holidays, increasing to 33 days with long service.
