Responsibilities Facilitate effective discharge planning to ensure patients transition smoothly from hospital care to home. Collaborate with a multidisciplinary team to create and implement tailored discharge plans. Assess and evaluate patients’ physical conditions and progress, ensuring a high standard of care. Communicate with patients and families, providing education and support to navigate the discharge process. Document all patient assessments, plans, and progress comprehensively and accurately. Qualifications HCPC Registered Physiotherapist ready to hit the ground running. Degree or equivalent qualification in Physiotherapy, ensuring you have the expertise needed. Proven experience in the UK, NHS, or hospital settings as a Physiotherapist is essential for this role.
