Care Coordinator Prescription Clerk The care coordinator will be part of the Severnvale Primary Care Network (PCN). The role involves coordinating with healthcare professionals and non‑clinical staff to support patients' medication management, particularly in care homes and for those requiring medication compliance aids. The post holder will help tackle inequalities in health and social care, support safe and efficient repeat prescribing, optimise medicines use, reduce waste, and maximise patient outcomes. Key Responsibilities Process repeat prescriptions daily, ensuring compliance with Prescription Hub protocols and SOPs within the required timeframe. Answer prescription queries from patients, carers, GP practice staff, community pharmacy staff, appliance suppliers, care home staff and other professionals. Maintain repeat medication records, including reconciliation, synchronisation and removal of outdated medication as guided by pharmacy technicians/pharmacists. Liaise with external services such as Community Pharmacies. Provide patients with clear, easy‑to‑understand medication information and support them in making informed choices. Proactively identify suitable patients through admission and discharge data. Work with patients, families or carers and the MDT to capture needs, support the development of Personalised Care Support Plans (PCSP), and highlight medication compliance issues. Attend and contribute to MDT meetings as required (e.g., care home residents). Keep accurate records in patient medical records and communicate progress or issues with managerial or clinical staff. Help identify a cohort of people and support personalized care requirements in partnership with GP practices and PCN staff. Provide excellent customer care, demonstrating empathy, patience and a holistic approach to patient care. Coordinate monitoring of long‑term conditions where appropriate. Support the delivery of the Quality and Outcomes Framework, enhanced services and other PCN service requirements. Participate in public health campaigns such as flu and Covid vaccination clinics. Understand safeguarding issues for vulnerable patient groups, including the elderly, housebound and those with long‑term physical or mental disabilities. Complete all mandatory training and induction programmes and attend formal appraisals with your manager at least every 12 months. Work weekends as required. Skills & Knowledge Essential Excellent communication skills. Knowledge of local health and social care services and providers. Proven record of team working. Ability to work under pressure. IT literate (MS Office). Desirable Understanding of health promotion issues and current local initiatives. Experience of EMISWeb clinical system. Adaptability to change. Qualifications Essential Minimum GCSE in English and Maths at C or equivalent. Personalised Care Institute eLearning: Core Skills, Shared Decision Making, Personalised Care and Support Planning (training to be completed on the job). Desirable NVQ Level 2 or 3 in Health and Social Care. Experience Essential Customer/patient contact experience with initiative and independent workload management. Desirable Experience in a practice, health and social care role or a pharmacy. Job Details Pay: £12.65 per hour. Contract: Permanent. Working pattern: Part‑time, 30 hours per week, Monday‑Friday 11:30‑17:30. Location: Almondsbury Surgery, Sundays Hill, Almondsbury, Bristol, BS32 4DS.
