Job Summary
The post-holder will assist the practice and Clinical Pharmacists in the improvement of patients’ health outcomes and the efficiency of the practice. You will work as part of a multidisciplinary team to coordinate medication changes on transfer of care, support the team with medication queries from clinicians and patients. You will work closely with local community pharmacies, secondary care and community teams to ensure seamless care for our patients.

You will be well supported by a team of Clinical Pharmacists to deliver evidence based, cost effective, safe and patient centred care.

You will support the practice with the medicines monitoring for patients which will involve running searches, keeping data bases up to date, contacting patients when they are due relevant blood tests.

**Responsibilities**:

- Support the role of the Pharmacy Team
- Support vulnerable patients (patients with learning difficulties/severe mental illness/memory impairment/dementia or where a patient lacks capacity or has no family member) with medicines management.
- Follow the process for when a patient is started on a new amber drug which requires monitoring from secondary care.
- Arrange bloods tests, add alerts to patient’s medical record and update prescription page.
- Inform patient of the monitoring procedure of the drug and frequency.
- Run searches within EMIS system to identify amber drugs that the practice should proactively be monitoring and send texts to patients who are overdue their bloods.
- Undertake medicines reconciliation in accordance with practice protocols and update patient’s medical records and communicate changes with patients where appropriate.
- Respond to medication queries from patients and staff in a professional manner working within your competence and seeking advice when required.
- Support achievement of the practice’s prescribing Quality and Outcomes Frames and Impact and Investment Fund.
- Promotion of electronic repeat dispensing and online ordering
- Undertake, participate, and share the outcomes of audits to facilitate improvements in medicines monitoring and take the lead role in rolling out any changes.
- Take a proactive approach to the safe handling of medicines and prescriptions, including assessment of risk and making recommendations for improvement.
- Use and maintain information systems and databases relevant to the position including incident reporting, medicines optimization, prescribing data, cost savings and patient outcomes.
- Participate in practice meetings, patient participation groups, and other meetings to improve engagement of the role of Medicines Management Care coordinator within the practice.
- Liaise with other pharmacy stakeholders as and when appropriate.
- Handle function specific information, which may be sensitive, complex, or confidential and appropriately recording, transferring and/or coordinating such information in accordance with the Data Protection Act; Caldicott Guidelines and the Confidentiality Code of Conduct
- To be responsible for the organisation, planning and of own workload to meet set deadlines.
- To utilise GP practice clinical systems to conduct regular searches for medicines monitoring.
- Support Pharmacist Lead with MHRA drug alerts, where appropriate and within level of competence, are acted upon within practices over an appropriate time frame and to advise on any medication changes that occur as a result.
- To assist in the monitoring of prescribing expenditure at individual practice level and propose changes to ensure containment within identified resource.
- To undertake learning and development to ensure the required knowledge and skills for practice-based work.
- Support the Pharmacy team with medication switches in line with practice and ICB agreed specific protocols - reviewing patients’ records, amending records and informing patients.
- To carry out other duties which are appropriate to the skills and competencies of the post holder and grade of the post as the priorities of the service change.

**Other Responsibilities**

**Administration**
- Contributes and participates in audits, evaluation and clinical standard setting within the Practice and PCN as required.
- Complete all required paperwork for legal and administrative purposes in accordance with relevant standards
- Ensure that all practice policies are fully implemented
- Work in accordance with all governance and internal systems relating to (but not limited to) the management of clinical data and systems
