**Summary**
Patients who undergo coronary artery bypass (CABG) surgery commonly experience angina and/or myocardial infarction within 10 years of surgery. The main reason is occlusive saphenous vein graft disease. PROTECT (randomised double blind Placebo-contROlled Trial of AdTIMP3 to prEvent Coronary artery bypass grafT failure) is a first-in-human advanced (gene) therapy clinical trial programme to prevent failure of coronary artery bypass grafting using autologous saphenous vein.
Scientists in our team have developed a gene therapy via adenoviral-mediated over-expression of “tissue inhibitor of metalloproteinases-3 (TIMP-3)” which has been shown to reduce neointima formation in human saphenous vein ex vivo and in short (28 day) and longer term (3 months) pig vein grafts in vivo. Our team has developed AdTIMP-3 according to Good Manufacturing Practice (GMP) standards and this therapy has been assessed in pre-clinical pig study. The next phase is clinical, including a phase I first-in-human study (n=12 patients) followed by a phase 2 randomised controlled trial in approximately 120 patients to assess the safety and efficacy of this novel therapy in patients undergoing CABG surgery. This is a bench-to-bedside academic collaboration from the University of Glasgow, NHS Greater Glasgow and Clyde Health Board (Co-sponsors) and the NHS Golden Jubilee hospital. This research programme is funded by the Medical Research Council and the British Heart Foundation.
