Job Title: Social Worker – Adult’s Mental Health Hospital Discharge TeamLocation: SheffieldRate: £30.00An exciting job role has recently become available at the Sheffield City Council. They are looking for a new member for their Adult’s Mental Health Hospital Discharge Team.  What will your responsibilities be? Undertake social care assessments and deliver timely care packages for hospital inpatients prior to discharge. Conduct mental capacity assessments, make best interest decisions, and handle safeguarding as required. Attend 117 meetings, ward rounds, and professional meetings. Facilitate learning opportunities for ASYEs through shadowing and participate in the duty rota. The successful candidate will be welcomed and integrated into the team, receiving regular supervision and support.
