An exciting opportunity has arisen within the Adult Hospital First Contact Service for 3 full time Social Care Practitioners to join the Hospital Discharge Review Team on a permanent basis. We are looking to recruit a self-motivated individual with exceptional time management skills, the ability to manage a caseload independently and a desire to make a positive impact on people’s lives. The work pace is fast and rewarding.
The Hospital Discharge Review Team provides excellent support to a diverse range of people within Sheffield. The team visits people in their home after their discharge to assess and review long term care and support needs to ensure they continue to effectively meet an individual’s needs. You will work with a range of people with varying presenting needs including people who have physical disabilities or other complex needs.
This role will enable you to make a real difference to the lives of Sheffield people, by offering comprehensive Care Act assessments, support planning, reviews, safeguarding and assessing mental capacity. The focus is to keep the individual in their own home and enabling people to be as independent as possible and being creative with assistive technology.
A strength-based approach is required to promote independence and enhance quality of life. When long term needs are identified, your role will involve connecting the person to their community and engaging with other professionals in a timely way to achieve the outcomes that are right for the individual. You will also work with carers and refer for assessment of their needs in accordance with the Care Act 2014.
Experience of assessing under the Care Act 2014, Mental Capacity Act 2005 and safeguarding are desired. Skills and knowledge in recording, report writing and IT skills will be required but additional training will be provided for specific systems.
You will work remotely with an office base at the Northern General Hospital where you will come in once a week for office team day. You will be part of a supportive team and given opportunity to further develop in your career within Adult Social Care.

Full-time employees work 37 hours for 52 weeks of the year and we offer a generous holiday entitlement. We are open to discussions about a wide range of flexible working opportunities which benefit you and the Council, including reduced hour contracts, working part time or as a job share.
If you are appointed to this role, your starting salary will be at the bottom of the grade. If you provide payslip evidence that your basic pay is greater than the bottom of the grade, we will consider starting you at the point of minimum advantage within the grade.
