Connect2socialwork are urgently looking for an EHC Coordinator to join Medway with a Pay rate up to £320 a day on a locum basis.

Fully Remote workingJob Description;

Hold an allocated case load and be accountable for the statutory processes for this case load.
Working on complex cases and ensuring cases are taken to the correct decision-making groups for approval
Reduce the number of EOTAS cases for post 16 young people
Supporting workflows and completion of Post 16 transition plans - including amending and finalising plans and updating synergy and case management
Processing cases that are 'moved in' and 'moved out'.
Attendance to and arranging of meetings regarding pupils inc. arranging urgent professional's meetings and annual reviews for those not on roll at a provision
Ensuring synergy database is up to date and participating in all data cleansing activities.
Liaising with and ensuring excellent communicating with providers, other agencies and authorities and parents.
Conducting consultations for placements including following up on responses, updating synergy, issuing challenge and direction letters in consultation with Area Inclusion Lead/ Dep. Area inclusion Leads and Head of Inclusion, submitting case discussion form for independent provisions to the placement decision making group.
Communicating outcomes of decisions and updating synergy accordingly
Work with our commissioning partners for tuition referrals and independent setting referrals.
Maintaining the student files and accuracy of data recording

Requirements as a Social Worker include:

SWE Registration
Eligible to work in the UK
Drive and have access to a car
Post-qualified experience
