Main duties of the job This role requiresthe post holder to have a flexible approach to ensure the holistic needs of thepatient and their carers are met.
The support that you would be required toprovide includes personal care visits, emotional support for our patients and theircarers and respite sits.
The post holder willusually work in the community but may, on occasions, be required to supportdelivery of care on our in-patient unit at our Whittington site or otherhospice locations.
About us St Giles Hospice is a registered charity founded in 1983 by the Reverend Paul Brothwell, originally to improve the care of local people dyingwith cancer.
We now support people in our community whose illnesses may not be curable such as cancer, motor neurone disease, heart failure, MS and respiratory disease.
We also offer support for families and helpers.Our dedicated team provides individually-tailored care either at the hospice or in patients own homes across our communities.The range of specialist services is provided free of charge to those who need it.
Although St Giles receives some funding from the Government, we rely heavily on donations and fundraising.
Job description Job responsibilities The role entails but is not limited to: -Providing direct patient care in accordance with the District Nurse care plan and/or instructed by an appropriate healthcare professional.
Liaising with other health care professionals involved in the patients care is therefore an integral part of this role.Assisting the patient in the activities of daily living which they would normally perform for themselves if able.Providing physical care with dignity and respect for the individual ensuring their safety at all times.Protecting the patients right to confidentiality at all times.Providing support and guidance to family and carers to enable them to gain knowledge to care for the patient and to access relevant servicesProviding a listening ear and a supportive presence to patient, family and carersRespecting the persons home as a working environmentMaintaining accurate documentation of care given and communicate any change in the patients condition as appropriate.Maintaining IT competency in order to make full use of mobile phones, e-mail, the internet and on-line training.Accessing patient information via SystmOne to ensure up-to-date knowledge of the patients needs.Updating and input accurate documentation of care provided and patient information onto SystmOne.
Full training will be given on using this system.Exercising responsibility by accessing and accepting opportunities for clinical supervision and training and attend annual mandatory training.Ensuring regular communication and liaison with line manager as part of delivering an appropriate and high quality service including attendance at team meetings.Just so you know:We may review applications before the applicationreview date, however, if you apply after the application review date, yourapplication may not be considered.
