You will work autonomously, assessing, diagnosing and managing a range of acute and chronic conditions.
Your expertise will contribute to improving patient outcomes and ensuring the smooth delivery of care in the surgery.

We are a large GP Practice with 13,000 patient based in Knottingley with a branch surgery in Eggborough.Ash Grove Medical Centre is led by four GP Partners working along side the Practice Manager, supported by our clinical and non clinical team.
Job description Job responsibilities Key responsibilities:Conduct clinical assessments, examinations and diagnostic testsDiagnose and manage acute and chronic conditions in patientsPrescribe medications and treatments plans in line with your scope of practiceProvide expert advice, education and support to patients and familiesCollaborate with GPs and other healthcare professionals to ensure integrated careContribute to improving care pathways and patient experienceMain Duties and ResponsibilitiesEnsure that confidentiality is maintained at all times, with a duty not to disclose any confidential information after the employment ends.To work independently using own initiative and is accountable for own professional actions.To assess, diagnose, plan, implement and evaluate treatment/intervention and care for patients present with an undifferentiated diagnosis.To assess, diagnose, plan, implement and evaluate treatment/ intervention and care for patients with complex needs.Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long term condition.Clinically examine and assess patients needs from a physiological and psychological perspective and plan clinical care accordingly.Diagnose and manage both acute and chronic conditions, integrating both drug and nondrug based treatment methods into a management plan.Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence based practice and national and practice protocols and within scope of practice.To work with patients to support compliance with and adherence to prescribed treatments.Provide information and advice on prescribed or over the counter medication on medication regimens, side effects and interactions.Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations including initiation of effective emergency care.Support patients to adopt health promotion strategies to promote healthy lifestyles and apply principle of selfcare.Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for mental health.Carry out the partners instructions and requests.All staff are required to take their share in providing cover, when necessary, for colleagues who may be absent as a result of holidays or illness.Maintain a smart appearance and be helpful wherever possible.All staff should be conversant with and adhere to all written guidelines and protocolsprovided and available in particular: The Health and Safety Manual, the In-house Complaints Procedure, the Fire/Alarm Procedures and all protocols that relate to their specific role.Extra Duties and ResponsibilitiesTo produce accurate, contemporaneous and complete records of patients consultation, consistent with legislation, policies and procedures.Assess effectiveness of care delivery through self and peer review, benchmarking and formal compliance and waste disposalDeliver care according o NSF,NICE guidelines and evidence based care.Maintain practice effective communication within practice environment and with external raise any significant events and discuss learning opportunities in clinical and MDT outstanding QOF and arrange for this to be with colleagues and ensure hand over is carried patient identifiable data is locked away at the end of the day.Organisational and Statutory RequirementsThe Post Holder must be aware of the responsibility placed on him/her under the Health & Safety at work Act 1974 and ensure that agreed safety procedures are carried out in order to maintain a safe environment for employees, patients and employees should understand that it is their personal responsibility to comply with all other organizational and statutory requirements Confidentiality, Equal Opportunities, Child Protection, The In-house Complaints Procedure, Fire Alarm Protocols and all protocols that relate to their specific role etc.Understand and apply legal issues that support the identification of vulnerable and abused children and adults and to be aware of statutory/ vulnerable patients health procedures and local guidance.Confidentiality:In the course of seeking treatment, patients entrust us with, or allow us to gather, sensitive information in relation to their health and other matters.
